Application Process
Under the Passports Act 1992, you must be a New Zealand citizen to be issued with a New Zealand passport. The Passport Application Form covers:
Proof of New Zealand citizenship
Proof of name change details
Proof of your identity
Photographs (two identical photos)
Application fees

The Passport Application Form must be completed in full if:

* You are applying for a passport for the first time;
* You wish to replace your current passport;
* Your passport has expired; or
* Your passport has been lost or stolen.
A good beginning makes a good ending
